FORM mattresses come with a 365-night trial. Bedding and bed bases are final sale once delivered.

Mattresses: 365-night trial

Your trial starts the day your mattress is delivered. Sleep on it for at least 30 nights. If it isn’t right after that, contact us before night 365 and we’ll schedule a free pickup. No repackaging, no return fees.

We refund your original payment method within 5–10 business days after pickup, minus the value of any non-returnable promotional gifts. Shipping charges, if any were paid, aren’t refundable.

The trial covers mattresses bought at formsleep.com that are in usable condition, free from damage caused by misuse, stains that void the warranty, or material alteration. Mattresses bought from another retailer follow that retailer’s return policy.

Bedding and bed bases: final sale

Pillows, sheets, mattress protectors, adjustable bed bases, foundations and headboard bracket kits are final sale and can’t be returned or exchanged once delivered. Not sure about size or fit? Chat with a FORM sleep specialist before you order.

Arrived damaged or defective? Contact us with your order number and a photo, and we’ll replace the damaged part or the product at no cost to you.

Warranties

Every FORM mattress is backed by a Lifetime Warranty for the original purchaser. The full terms are in section 8 of our Terms & Conditions.
