Arrived damaged or defective?
Contact us with your order number and a photo. We’ll replace the damaged part or the product at no cost to you.
Warranty coverage

| FORM Upholstered Foundation | 1-year limited. Covers manufacturing defects and faults in the upholstery. |
| FORM CryoPhase Pillow | 10-year limited. Covers defects, including foam that splits, cracks or stops returning to its shape. |
| FORM TENCEL™ Sheets | 3-year limited. Covers manufacturing defects such as faulty seams or flawed stitching. |
| FORM Mattress Protector | 5-year limited. Covers manufacturing defects. |
FORM Adjustable Bed Base warranty
- Year 1: we cover repairs and replacements, including parts, labor and shipping.
- Years 2–5: replacement parts are covered. You cover service and shipping, and defective parts must be returned within 14 days.
- Years 6–10: replacement parts are available at a prorated cost based on the months since year 5.
What’s not covered
Normal wear and tear, gradual softening, fading, pilling and shrinkage; damage from misuse, improper care or exceeding the weight limit; unauthorized repairs; and products bought from unauthorized sellers. Warranties cover the original purchaser.
